Safety of Patients in New Zealand Hospitals: A Progress Report
In April 2007 the Commissioner asked all district health boards: “What safeguards are in place to prevent a case like the tragic death of a 50-year-old at Wellington Hospital in September 2004, occurring at your hospitals?”
Dr Seddon's full report “Safety of Patients in New Zealand Hospitals: A Progress Report” is now available. It includes her reviews of individual DHB responses.
The linked documents below are each DHB's initial response and (where applicable) the supplementary response from that DHB (listed alphabetically). It is important to note that the initial responses from each DHB were not designed to give the full picture of all the quality improvement activites under way to ensure the safety of hospital patients. The review by Dr Seddon has been a paper review of the information supplied by each DHB, and must be read in conjunction with the supplementary information provided by that DHB.
