What is a Health Passport?

A Health Passport is a booklet that you carry with you when you visit health and disability services. A Health Passport has information about how you want people to communicate with you and support you when you use health or disability services.

The Health Passport is not a tool to help doctors diagnose or monitor patients. It is not a substitute for a patient’s medical records.

How do I get a Health Passport?

You can get the Health Passport in either A5 or A4 formats. It is also available in Easy Read. Click on the button below to download a copy of the Health Passport. You can also contact HDC and we will send you one for free.
